Our Founder

Dr. Behn Banihashemi, MD FRCPC FACC, is a compassionate, double board–certified cardiologist with advanced training in echocardiography and nuclear cardiology and over 10 years of clinical experience. A graduate of the University of Ottawa Faculty of Medicine, he completed his Internal Medicine residency at the University of Toronto and fellowships in Cardiology and Echocardiography at the Ottawa Heart Institute. With a strong interest in aging, age management, and holistic preventive medicine, he provides comprehensive, personalized, and high‑quality care grounded in extensive clinical experience.

Dr. Nicole Zito, MD FRCSC
Dr. Nicole Zito MD FRCSC is a board-certified specialist in Obstetrics and Gynecology. She completed medical school at McMaster University in 2015 and residency in Obstetrics and Gynecology at the University of Western Ontario in 2021. She also had additional training in family medicine. She is a member of the Menopause Society and the Canadian Menopause Society. She is also a certified Menopause Provider by the Menopause Society (formerly NAMS). She is passionate about preventive medicine and understands that healthcare is best delivered through a holistic approach. In her spare time, she stays active through a mix of Pilates, strength training, and jogging.

Juliana Choueiry, NP
Juliana Choueiry, MScN, NP, is a Nurse Practitioner at the Cardiac Longevity Clinic who is passionate about preventing heart disease and supporting long-term health. She brings a strong clinical background from her work as a Nurse Practitioner at the Children’s Hospital of Eastern Ontario, where she cares for patients with complex and acute medical needs. With a solid foundation in clinical research, Juliana uses evidence-based care to focus on cardiovascular risk reduction, lifestyle habits, and early intervention. Juliana is passionate about empowering individuals with the knowledge and tools needed to support long-term heart health and overall well-being.
Sylvia Girgis, RN
I am a Registered Nurse that holds two bachelor’s degrees: a Bachelor of Science in Biology with Health Sciences Honours from Carleton University, and an Accelerated Bachelor of Nursing from Queen’s University. I am trilingual and bring strong communication skills to my practice. I am passionate about delivering compassionate, patient-centred care and advocating for patients to ensure their voices are heard and needs are met. I value collaboration, empathy, and continuous learning in healthcare. Outside of work, you will find me catching up on my favourite TV shows, doing yoga, spending quality time with friends playing board games or travelling and exploring the world.

Georgia Cunningham Executive Assistant
Georgia holds a BASc in Human Behaviour from McMaster University and brings several years of experience in customer-facing roles and medical administration to her role as the clinic’s Executive Assistant. Working closely with both clients and the clinical team, she ensures each client’s journey is well supported and seamless. Georgia is passionate about helping others achieve their goals and values a personalized, preventative approach to care. Outside of the clinic, she enjoys spending time outdoors and reading.

Andrew Holmes is a Registered Polysomnographic Technologist (RPSGT) and leading sleep specialist with over 20 years of experience in sleep diagnostics and therapeutic solutions. Highly regarded in his field, he consults for professional sports organizations, including the Ottawa Senators of the National Hockey League. Andrew has appeared on CBC’s The National as a sleep subject matter expert, along with numerous other media appearances. His career includes senior leadership roles in hospital and healthcare settings, and the founding of Sleep Efficiency Inc. in 2015, now Canada’s premier home-based sleep testing service. Andrew continues to advocate for optimal sleep health in the community, and with Sleep Efficiency, he has created a distinguished record of providing comfort to individuals by conducting sleep tests in their own homes.

Debora Sloan, RD, CSSD, CPT
Debora Sloan is a Registered Dietician, certified Sport Nutritionist, and certified Personal Trainer. She completed a Bachelor of Arts (Honours) at Queen’s University, followed by a Bachelor of Science in Clinical Nutrition and Dietetics from Western University, and received clinical training at Sunnybrook Hospital in Toronto. Debora established her private practice in 2013. As an athlete and mother living with Type 1 diabetes and celiac disease, she understands the challenges of balancing a busy lifestyle with healthy living and proper nutrition. With expertise in diabetes, heart health, metabolic disorders, weight management, sport nutrition, celiac disease, aging, and women’s health, Debora combines extensive professional knowledge with lived experience to create individualized, sustainable nutrition and fitness solutions for clients of all ages and stages.

Free Form Fitness is proud to partner with Cardiac Longevity Clinic to support clients looking to improve their long-term health, strength, and overall well-being. As part of the clinic’s full health assessment, we provide thorough fitness assessments that measure mobility, posture, balance, and strength. These assessments create a clear starting point and guide personalized exercise recommendations that align with each client’s medical needs and lifestyle goals. Our personal training programs are grounded in a longevity-focused, functional training philosophy. We prioritize sustainable strength, improved movement quality, and exercises that support real-life function, independence, and long-term health. We offer efficient 30-minute private sessions that deliver personalized, results-driven coaching. Clients can train at any of our four Ottawa locations: Wellington Village, Kanata, Glebe, and Downtown.

A specialized cardiovascular assessment designed for women navigating perimenopause and menopause. We recognize the unique hormonal and metabolic changes that occur during this stage of life, and understand the associated cardiovascular risk. Through this proactive assessment, we aim to help clients feel confident in managing symptoms and well informed when considering hormone therapy.
Through comprehensive clinical evaluation, clients undergo a personalized risk assessment based on their symptoms, medical history, and goals. Where appropriate, advanced cardiac testing is integrated to support early detection and prevention. Together, we provide our clients with individually tailored, evidence-based guidance to protect their heart health during midlife and beyond.
